Medical Description

Calcium channel blocker used in the treatment of chronic hypertension, angina pectoris and Raynaud’s disease.

Indication & Usage

-Chronic hypertension: Oral: Initial: 2.5 to 5 mg once daily; evaluate response after ~2 to 4 weeks and titrate dose, as needed, up to a maximum of 10 mg once daily; if additional blood pressure control is needed, consider combination therapy. -Angina pectoris: In case of chronic stable angina: Oral: 5 to 10 mg once daily. While in case of vasospastic angina: Oral: 5 to 10 mg once daily. -Raynaud’s disease: Oral: 5 mg once daily; if needed, increase dose gradually based on patient response and tolerability, usually once every 4 weeks and monitor blood pressure closely with each dose increase; maximum dose: 20 mg/day. -Pediatric dosing: -Hypertension: Children <6 years: Limited data available: Oral: Initial: 0.1 mg/kg/dose once daily; titrate based on clinical response; maximum daily dose: 0.6 mg/kg/day or 5 mg/day. While in case of children ≥6 years and adolescents: Oral: Initial: 2.5 mg once daily; titrate based on clinical response; maximum daily dose: 10 mg/day. -Raynaud’s disease: Very limited data available: Children ≥6 years and Adolescents: Oral: Usual dosage range: 2.5 to 10 mg once daily.

Administer without regard to meals.

Side Effects

Peripheral edema, Flushing, palpitations, pruritus , skin rash, abdominal pain, nausea, dizziness, drowsiness, fatigue, male sexual disorder, asthenia, muscle cramps, dyspnea, peripheral ischemia, sinus tachycardia, syncope, vasculitis, diaphoresis, erythema multiforme, hot flash, hyperglycemia, weight gain, weight loss, anorexia, constipation, dysphagia, flatulence, gingival hyperplasia, pancreatitis, vomiting, xerostomia, difficulty in micturition, nocturia, urinary frequency, leukopenia, purpuric disease, thrombocytopenia, angioedema, hypersensitivity reaction, abnormal dreams, anxiety, depersonalization, depression, female sexual disorder, hypoesthesia, insomnia, malaise, pain, paresthesia, peripheral neuropathy, rigors, vertigo, arthralgia, back pain, myalgia, osteoarthritis, tremor, conjunctivitis, diplopia, eye pain, tinnitus and epistaxis.

Safety Advice

-Should not be given to patients suffering from hypersensitivity to amlodipine. -Use amlodipine with extreme caution in patients with severe aortic stenosis. -Use with caution in patients with hepatic impairment. -Use amlodipine with caution in patients with hypertrophic cardiomyopathy and left ventricular outflow tract obstruction. -In case of over dose be ready to tell or show what was taken, how much and when it happened, and seek immediate medical attention. Drug Interactions

-Alfuzosin: May enhance the hypotensive effect of blood pressure lowering agents. -Antipsychotic Agents (Second Generation [Atypical]): blood pressure lowering agents may enhance the hypotensive effect of antipsychotic agents. -Calcium Salts: May diminish the therapeutic effect of calcium channel blockers. -Carbamazepine: amlodipine may increase the serum concentration of carbamazepine. -Clopidogrel: Calcium Channel Blockers may diminish the therapeutic effect of Clopidogrel. -Dabigatran Etexilate: Amlodipine may diminish the therapeutic effect of Dabigatran Etexilate. -Dantrolene: May enhance the hyperkalemic effect of Calcium Channel Blockers. -Dapoxetine: May enhance the orthostatic hypotensive effect of Calcium Channel Blockers. -Lovastatin: Amlodipine may increase the serum concentration of Lovastatin. -Magnesium Sulfate: May enhance the adverse/toxic effect of Calcium Channel Blockers. -Melatonin: May diminish the antihypertensive effect of Calcium Channel Blockers. -Red Yeast Rice: Amlodipine may increase the serum concentration of Red Yeast Rice. -Simvastatin: Amlodipine may increase the serum concentration of Simvastatin.

Pregnancy & Lactation

-Amlodipine crosses the placenta. Calcium channel blockers may be used to treat hypertension in pregnant women; however, agents other than amlodipine are more commonly used. -Amlodipine is present in breast milk. The relative infant dose (RID) of amlodipine is 4.18%, In general, breastfeeding is considered acceptable when the RID is <10%. -Ask your physician before taking any medications during pregnancy and lactation.
