Medical Description

Erythropoiesis-stimulating agent (ESA); used in the treatment of anemia due to chronic kidney disease and anemia due to chemotherapy.

Indication & Usage

indicated for the treatment of anemia associated with: Chronic kidney disease (CKD) (on dialysis or not) Chemotherapy-induced anemia in cancer patients Anemia due to zidovudine therapy in HIV patients Reduction of transfusion requirements in selected surgical patients

Dosage & Administration

Administered by subcutaneous (SC) or intravenous (IV) injection CKD (adults): Typical starting dose: 50–100 IU/kg, 3 times weekly Dose adjusted based on hemoglobin response Target hemoglobin usually 10–11.5 g/dL Ensure adequate iron stores before and during therapy

Side Effects

Common: Hypertension Headache Injection-site reactions Arthralgia Serious: Thromboembolic events Rapid rise in hemoglobin Seizures (rare) Pure red cell aplasia (very rare)

Contraindications Hypersensitivity to epoetin alfa or excipients Uncontrolled hypertension History of antibody-mediated pure red cell aplasia Precautions Monitor blood pressure regularly Monitor hemoglobin every 1–2 weeks during initiation Avoid rapid hemoglobin increase (>1 g/dL in 2 weeks) Increased cardiovascular risk at higher Hb targets Use lowest effective dose
