Medical Description

Beta-1 blocker used in the treatment of hypertension, heart failure with reduced ejection fraction, angina and arrhythmia.

Indication & Usage

Hypertension Chronic stable angina pectoris Chronic heart failure (stable, NYHA III–IV, in combination with standard therapy)

Dosage & Administration

Hypertension / Angina: 5 mg once daily (range 2.5–10 mg/day) Heart failure: Start at 1.25 mg once daily, titrate gradually (1.25 → 2.5 → 3.75 → 5 → 7.5 → 10 mg) Take in the morning, with or without food Swallow tablet whole with water

Side Effects

Some of the following side effects may occur: Cough. Diarrhea or constipation. Dizziness or lightheadedness. Erection problems. Feeling nervous. Feeling tired, weak, drowsy, or a lack of energy. Headache. Nausea or vomiting.

Contraindications Hypersensitivity to bisoprolol Acute heart failure or cardiogenic shock Severe bradycardia Second or third-degree AV block Severe hypotension Severe bronchial asthma Precautions Do not discontinue abruptly Use caution in diabetes (masks hypoglycemia) Use cautiously in COPD/asthma Monitor heart rate and blood pressure Dose adjustment may be required in hepatic or renal impairment

Drug Interactions

Calcium channel blockers (verapamil, diltiazem) → ↑ bradycardia/heart block Digoxin → additive bradycardia Antiarrhythmics (amiodarone) → conduction disturbances Insulin/oral antidiabetics → masked hypoglycemia NSAIDs → reduced antihypertensive effect

Pregnancy & Lactation

Pregnancy: Use only if clearly needed; beta-blockers may cause fetal bradycardia, hypoglycemia, or growth restriction Lactation: Small amounts excreted in breast milk; use with caution and monitor infant
